INSTRUCTIONS FOR PROOF OF CLAIM AGAINST CASUALTY UNDERWRITERS INSURANCE COMPANY, IN LIQUIDATION

Important Notice

• If you filed a claim with Casualty Underwriters Insurance Company (“CUIC”) and your claim number begins with the letter C followed by seven digits (for example, C1234567), do not submit a Proof of Claim until you have fully pursued and exhausted your claim through your state’s guaranty association.

• If the Liquidator or the Notice of Liquidation directs you to file a Proof of Claim (POC), follow the instructions below carefully.

Completing the Claim Form

• Fill out the form as accurately and completely as possible. Incomplete or inaccurate forms may delay or result in denial of your claim.

• Attach additional sheets if you need more space.

• If your claim category is not listed, check “Other” and clearly describe the nature of your claim so it can be properly classified.

• Include supporting documentation such as contracts, invoices, undertakings, or other evidence with your claim.

Filing Methods

You may file electronically, by fax, or by mail/delivery.

• Upload supporting documents

• Submit and retain your confirmation and assigned Proof of Claim number

3. Mail or Delivery

• Print and mail or deliver the completed form and supporting documents to:

• If mailed, the envelope must be postmarked by 5:00 P.M. MDT on the bar date.

• If delivered, it must be received at the address above by 5:00 P.M. MDT on the bar date.

Claimant Responsibilities

• Once a Proof of Claim is submitted, it is your responsibility to notify the Liquidator of any changes to your name, EIN, email address, telephone number, or mailing address.

Deadline

• THE BAR DATE IS OCTOBER 20, 2026, AT 5:00 P.M. MDT

• Claims received after this date may be barred or reduced.

After You File

• Keep your Proof of Claim number and confirmation.

• Notify the Liquidator if your name, EIN, email, phone, or address changes.
